ORIGINAL -PUBLIC FILE <br />(Page 2) <br />MINE ID # OR PROSPECTING ID #: n~_19~_nl,t <br />INSPECTION DATE: - 9/~/n7 INSPECTOROS INITIALS: ~_ <br />OBSERVATIONS <br />The Division conducted a monitoring inspection of the site as part of sites to be <br />inspected during the Fiscal years 2007 & 2008. Present during the inspection was Tom <br />Smith of Rocky Mountain Materials. <br />1) Site is a large 112 Phased sand and gravel operation consisting of 2,496 acres in <br />two areas The old parcel, North of State Highway 120 and the active portion South of <br />the high way. <br />2) The portion of the permit south of the high way was part of the Amendment that was <br />added after the operator purchased the parcel of land, even though divided by the <br />highway, the Division approved the permit. <br />3) The portion of the old permit area North of the State High Way 120, all mining has <br />been completed. According to Mr. Smith, once the existing processed stockpiled <br />material is removed from the area, reclamation will commence. The site also consists <br />of a 5 bedroom house, which is occupied by one of Rocky Mountain Material <br />employees.. In anticipation of placing some live stock on site, there was a small <br />stockpile of bales of hay, along the west side of the house. <br />4) Continued the inspection to the active mining area. It was active at the time of the <br />inspection. Large quantities of processed stockpiled materials were noted scattered <br />through out the affected acres. There was a conveyer system measuring approximately <br />800 feet searching North South along the active mining area. Pit high walls were <br />approximately 10-13 feet in height and stable. <br />5) All the drainage from the operation is routed from the affected acres to three large <br />series settling ponds below the affected acres. The ponds were dry with exception of <br />the lowest pond, which is the clean water pond. The water from the lower pond is <br />utilized on the property for dust suppression and other uses. There was a small drag <br />line noted along the east side of the upper pond at the time of the inspection. <br />6) site is a phased 112 with maximum 100 acres disturbance at any given time. Financial <br />warranty was set at $200,812.00 in 2003 when the site was amended by adding 2,260 <br />acres to the permit. At that time the total disturbance was approximately 85 acres. <br />The annual reports do not appear to show the total current affected acres. The <br />Division at this time requests the operator to survey the total affected acres, and <br />anticipated areas to be disturbed in the coming year, so the financial warranty can <br />be adjusted accordingly. In the affected acres, please include the maximum height <br />and length of the high walls, maximum length of the conveyer system, and all <br />ancillary disturbances. Once the Division received the requested information, the <br />financial warranty will be adjusted accordingly. If there are any backfilled and <br />graded affected acres that the operator wants the Division to consider, please <br />include them in the report and maps. <br />7) Top soil stockpiles were properly stockpiled and protected, Permit sign was properly <br />posted at the access to the property. No noxious weeds noted. <br />18.
