(Page 2)
MINE ID # OR PROSPECTING ID #: M-i9Rn-~aa
INSPECTION DATE: >>-~-nn INSPECTORS INITIALS: _~~
The Division conducted an inspection of the site at the request of the operator. CC&V contacted the Division
and requested an on site meeting to go over the planned relocation of Highway 67 and the bridge that will
be needed to be constructed below the underdrain discharge point as part of the highway relocation.
Present during the site inspection were, Phil Barnes, Terry Mandziac and ]im Smith of CC&V. Drove to the
underdrain discharge area and the operators representative discussed two possibilities on how they plan to
transport the large crane that will be utilized to build the bridge to a location below the toe dam. One of the
options discussed was to construct a 40 feet wide road on portions of the toe dam. Since the toe dam was
constructed from fill material the Division registered some concerns about the stability and requested the
design engineer to provide the appropriate engineering calculation to show that the toe dam will not be
compromised by such activity. The operator's representatives agreed to provide the appropriate safety
calculations or study that will show that the toe dam will not be compromised by such activities. While at
the location inspected the underdrain discharge? Both 24-inch pipes and the four-inch pipe were
discharging. No measurements were taken. The operator also plans to extend the discharge pipes below the
brogue relocation area until the construction of the bridge is completed. The Division has no objection to
the extension of the discharge pipes. While in the area, the following readings were taken from the Phase II
Low and high volume transducers.
Low volume pump # i registered .06 feet of head and # 2 registered .07 feet of head.
HIGH volume pump # 4 registered 14.4 feet of head
#5 registered 19.1 feet of head
#6 registered 17 feet of head
The replacements of HDPE header pipes with steel pipes were wntinuing and no major problem noted.
I & E Contact Address
NAME: SCOTT LEWIS
OPERATOR: CC&V
STREET: P O.BOX 191
CITY/STATE/ZIP: ~IICTOR.
