.. <br />MINE ID # OR PROSPECTING ID #: M-iasnaaa <br />INSPECTION DATE: ~-S-m INSPECTORS INITIALS: -~1~ <br />OBSERVATIONS <br />The Division conducted a monitoring inspection of the Phase I through III leach pad areas and construction progress <br />inspection of the Phase IY areas on 3-5-02. Prior to the site inspection, a brief discussion regarding the EXACT <br />ELEVATION OF WHERE THE PSSA (PREGNANAT SOLUTION STORAGE AREA) WAS FOR BOTH PASE I AND II PREGANANT <br />PONDS.The discussion has been on going and both the mine (CC&V) and the Division wanted to make sure every one is <br />aware of the BO°lo operational capacity, Because the toe dam elevation During the Phase IV underground remediation <br />inspection, Bob Nickels of Golder associates the resident QA(QC lead person and Terry Mnadziac, of CC&V, who over sees <br />the project for the mine were present. Prior to the physical inspection, both Terry and Bob briefly discussed the progress <br />of the remediation work. They both stated that to date, there was no surprise under ground workings they found that was <br />not expected. Observed the underground remediation work in the Phase IV PSSA storage area. No major problem noted. <br />Inspected the external pond, as reported by Phil Barnes the week of the eighteenth of February, the low volume solution <br />collection pump froze up due to extreme cold temperatures the area experienced. Since the pond is holding only storm <br />water and the certification approval letter has not been sent, CC&V plans to pull the pumps out and rectify the problem so <br />it will not be repeated. At the time of the inspection the one transducer was recorded at 97 inches of hydrostatic head. <br />Inspected the Phase II pregnant solution collection system and recorded the following readings from the High and Low <br />volume solution collection transducers. <br />High Volume Transducer # 4 was recorded at 29.2, <br />#5 was recorded at 36.0 <br />#6 was recorded at 43.3 <br />Low volume Transducer #1 was recorded at .14 <br />#2 was recorded at .25 <br />Inspected the new crusher area. Work was continuing on the LOB AND secondary crusher.
