(Page 2) <br />.MINE ID # OR PROSPECTING ID #M-80-244 <br />INSPECTION DATE 11-12-99 <br /> <br />INSPECTOR'S INITIALS HK <br />The Division conducted a monitoring inspection of the area on 11-12-99.P resent during the <br />inspection was Mr. Scott Lewis of CC&V. The inspection began near the Phase II toe Dam area <br />where Mr. Lewis pointed out where they plan to get the material that will need to construct <br />the height increase for the toe berm. Since the burrow area is within the affected area, <br />there will be no need for additional warranty. Inspected the Phase II pregnant solution area. <br />The header pipe located at south of the pumps was being cut so it can be connected to the <br />Phase III expansion area. No problem noted. Mr. Lewis explained how the pipe was going to fit <br />in the existing area which would require placement of a small sump on top of the pregnant <br />pond. While in the area, the following readings were taken from the LOW volume and HIGH <br />volume collection pump transducers. Low volume pump #1 had .43 feet of head, Low Volume pump <br />#2 had .19 feet of head. High volume pump # 4 had 18.2 feet of head, pump #5 had 19.2 feet <br />of head and pump #4 had 23.5 feet of head. <br />Inspected the phase III permit area that was not covered under the certification report, work <br />was continuing on the backfilling and grading of the area. No underground remediation work <br />is expected until next year's construction season. Inspected the temporary diversion ditch <br />for the uncertified portion of phase III. The operator has placed two 48" CULVERTS APPROX. <br />SOFEET SOUTH OF THE INTERFACE OF THE CERTIFIED AND UNCERTIFIED AREAS. No problem noted. <br />Inspected the certified portion of Phase II and III AREAS. Ore deployment and leaching was <br />taking place at the time of the inspection. After the inspection, a brief discussion with Mr. <br />Terry Mandziac was held regarding the pending construction of the toe dam, under drain <br />extension and other minor construction activities that the operator plans to implement on <br />site prior to the end of the construction season.
