MINE ID # OR PROSPECTING ID #: 5-3I-ro <br />INSPECTION DATE: S-~t-ro INSPECTOR'S INITIALS: ~- <br />The Division conducted a monitoring and construction progress inspection of the Phase IV (A) AREA on 5- <br />31-02. Present during the inspection were, Tim Comer and Mark Tdquist of CC&V. The main purpose of the <br />inspection was to inspect the geomembrane lined area of the Phase IV (A). Approximately 200,000 square <br />feet of material had been deployed. The lined area was walked and inspected, no major problem noted. <br />In anticipation of geomebrane deployment, clay liner was being placed and compacted along the East Side <br />of the gemembrane lined area. Golder QA/QC personnel were present over looking and conducting the <br />required testings on the liner. <br />Continued the inspection to the phase II pregnant solution collection systems and recorded the following <br />readings from the High and Low volume solution collection transducers. <br />High Volume solution collection Transducer # 4 was recorded at 8.5 feet of hydrostatic head <br />#5 was recorded at 14. 7 feet head <br />#6 was recorded at 29.5 feet of head <br />Low volume solution collection transducer # 1 was recorded at .12 feet of head <br /># 2 was recorded at .17 feet of head <br />No major problem noted. <br />I & E Contat Address <br />NAME: ~s ~rf I L {SA-ICA.f h',S' <br />OPERATOR: CC&V <br />STREEI-: P.o.
