(Page 3) <br />MINE ID # OR PROSPECTING ID #M-1979-205 <br />INSPECTION DATE 5/20/03 <br />INSPECTOR'S INITIALS GRM <br />The following problems (PB) and/or possible violations (PV) (and suggested corrective actions) were identified during this <br />inspection. The problems should be corrected by the dates given, or they will become possible violations. The possible <br />violations should be corrected by the dates given to reduce their severity when considered by the Mined Land Reclamation <br />Board. The inspector noted on the previous page should be notified of all corrective actions taken. <br />PROBLEMS/POSSIBLE VIOLATIONS <br />AND <br />CORRECTIVE ACTIONS <br />CORRECTION <br />DATE <br />1. Mining has occurred beyond the 4- acre limit approved in-1998 in Phase II and III. 07 / 31 / 03 <br />Corrective Action: Submit a Technical Revision to the Mining Plan by the correction date noted to <br />reflect the acreage mined to date and operator s estimate of the amount of fill material available to <br />backfill the approximately 16.1 acres currently excavated. If the backfill material is insufficient, the <br />operator is to identify a source and cost for the additional materials necessary to complete the job. <br />2. A portable fuel tank was observed sitting on bare earth with no secondary containment. <br />Corrective Action: Submit Photo Documentation to this Office that the tank has been rp o°erly stored <br />and anv contaminated soil has been properly cleaned up by the Correction Date. Please note the <br />short time for compliance. <br />3. The Financial Warranty is inadequate to cover the costs for backfilling the current pit. <br />Corrective Action: Staff will complete an estimate of the costs to backfill the oit per current conditions. <br />If United /Old Castle has anv documentation to show the Water Augmentation Plan is near approval <br />copies should be sent to this Office as soon as possible.
