(Page 2) <br />MINE ID # OR PROSPECTING ID #: ~~-~ aax-n25 <br />INSPECTION DATE: n~/~ 4/11 INSPECTOR'S INITIALS: ~!? <br />OBSERVATIONS <br />This inspection was conducted as part of the Division of Minerals and Geology's four-year monitoring <br />program. Present at the inspection were Kate Pickford, DMG, and Dennis Good, R E Monks <br />Construction, Co. Weather at the time of the inspection was cloudy, windy, and approximately 20 <br />degrees. <br />2. A sign is posted at the entrance to the site and the permit boundaries are marked with fencing. There is <br />presently no active mining occurring at the site. All disturbance at the site occurred prior to he <br />Colorado Land Reclamation Act. The historic disturbance at the site is all vegetated. In that the permit <br />was issued March 19, 1986 and no mining has occurred at the site since issuance of the permit, if the <br />operator does not intend to commence mining of the site during 2006, the operator should request that <br />the site be placed into temporary cessation status or request a release of the site(see problem 1, page <br />3) <br />3.
