This inspection was in response to a permit application for a new 1 12c permit and serves as the Pre- <br />operational inspection for the site. Present at the inspection were Kate Pickford, DMG, and Matt <br />Carnahan. Gravel, LLC. Weather at the time of the inspection was clear, calm, and approximately 70 <br />degrees. <br />2. The proposed affected area of the site is currently agricultural and pastureland with some wetlands. In <br />addition, the site contains an irrigations ditch, fences, utility poles and lines, and access roads. <br />3. The Florida River runs through the proposed permit area. The submitted application materials state that <br />a 60-foot setback will be maintained from the river. Mr. Carnahan stated that there would actually be <br />a 100-150 foot setback from the river. The Division will need information indicating whether the <br />proposed mining area falls within the 100-year flood plain of the river. This issue was addressed in the <br />adequacy review letter (see item 7). <br />d. During the inspection, an infestation of Canada thistle was observed at the site. The Applicant will need <br />to include a weed control plan with the permit application materials. This issue was addressed in the <br />adequacy review letter (see item 7}. <br />5. During the inspection, it was discovered that a portion of the proposed access road to the site is not <br />included in the proposed permit boundary. All access roads constructed for the project must be <br />included in the permit boundary. This issue was addressed in the adequacy review letter (see item 7). <br />6. Some wetlands are located on the site. Therefore a wetlands delineation study is required to identify <br />whether any of the wetlands are jurisdictional. This issue was addressed in the adequacy review letter <br />(see item 7). <br />7. Some of the information contained in the original application materials submitted to the Division for <br />review does not adequately address the information required by the Division. Additionally, conditions <br />observed during this inspection identified further deficiencies in the permit application. The <br />deficiencies have been addressed in the Adequacy Review letter dated August 30, 2005.
