(Page 3) • <br />MINE ID ~ OR PROSPECTING ID ~ M-77-081 <br />INSPECTION DATE 11/29/94 INSPECTOR'S INITIALS GSC <br />2. The mining and reclamation plane for this operation do not include the use of hazardous <br />substances. The operator recycles concrete and asphalt in Stage F, about 2,500 feet <br />northeast of the homes along 28th Avenue. The recycling area does not appear to represent <br />a threat to the residential wells since it is downgradient from the wells and at such a great <br />distance. <br />The following problems (PB) and/or possible violations (PV) (and suggested corrective <br />actions) were identified during this inspection. The problems should be corrected by the <br />dates given, or they will become possible violations. The possible violations should be <br />corrected by the dates given to reduce their severity when considered by the Mined Land <br />Reclamation Board. The inspector noted on the previous page should be notified of all <br />corrective actions taken. <br />PROBLEMS/POSSIBLE VIOLATIONS <br />AND <br />CORRECTIVE ACTIONS <br />CORRECTION <br />DATE <br />The Stage D dewatering trench has been extended to the 1/19/94 <br />southeastern portion of the permit, approximately 55 PB or PV PB <br />feet from power poles serving a residential area along TOPIC(S) OD <br />28th Avenue and 65 feet from West F Street. <br />Corrective Action: The operator will need to <br />demonstrate the mining operation will not adversely <br />affect the stability of significant permanent <br />structures within 200 feet, pursuant to C.R.S. 34-32- <br />115(4)(d). Alternatively, the operator may submit <br />verification of a signed agreement between the operator <br />and the owners of those structures which either allows <br />for mining within a designated distance or maintains <br />that any damage caused by the operation will be <br />compensated by the operator. Submit this to the <br />Division no later than 1/19/94. <br />2. Disturbance in Stages C and D do not coincide with the 1/19/95 <br />approved mining and reclamation plans.
