Long <br />Division Director <br />RE: Corrective Actions, March 22, 1995 Inspection Report, Tezak Construction <br />Company, Tezak Pit No, 2, File No. M-81-080 <br />Dear Mr. Klco: <br />It is the Division's understanding that you have been retained by Mike Tezak to help <br />him abate the problems noted in the March 22, 1995 inspection report. <br />I am enclosing all the forms you will need to meet the requirements of the various <br />corrective action items. Enclosed please find a 110 permit application package, a <br />transfer of permit and succession of operator application, a performance warranty <br />form, and a financial warranty corporate surety form. Please let Lavern Lemay in our <br />office know if you want to provide a different type of financial warranty (bond) so that <br />we can send the appropriate forms. I have also enclosed a copy of the <br />June 10, 1993 letter from the Division explaining what you must do since you have <br />changed your company name. <br />You requested a copy of the permit, Exhibit C Map. We had the map reproduced at <br />CTS Legal Services.
