(Page 2) <br />MINE ID # OR PROSPECTING ID # M-1980-085 <br />INSPECTION DATE 10-29-2002 <br />INSPECTOR'S INITIALS SSS <br />This was a routine partial inspection conducted as part of DMG s ongoing monitoring program for permitted mine sites. <br />The mine identification sign and affected area boundary markers are in place and in compliance with Rule 3.1.12. <br />No significant infestations of noxious or other problematic weeds were noted on site during this inspection. <br />No significant erosional features were noted on the affected areas of the site. <br />Fill wntinues to be imported to the site as permanent backfill material. The permit indicates that importation of all fill material <br />would include a signed affidavit certifying that the material is clean and inert, specifying it s origin and the estimated volume. <br />Please submit copies of these affidavits for the past year. <br />There were a few barrels in the asphalt batch plant area, containing hazardous materials, that were not stored properly within <br />the approved spill containment structure. These barrels, and any others containing hazardous materials, must either be stored <br />within approved spill containment structures or be removed from site and properly disposed of. These barrels are noted as a <br />problem, with corrective actions and a corrective action due date, on the last page of this report. <br />The reclamation cost estimate was not recalculated as part of this inspection. The current estimate was recalculated less than <br />three years ago and the operation is in compliance with the approved mine plan and reclamation plan.
