(Page 2) <br />MINE ID # OR PROSPECTING ID # M-1979-142 <br />INSPECTION DATE 09/18/2006 <br />INSPECTOR'S INITIALS DEG <br />This routine monitoring inspection of Permit Number M-1979-142 (West of Maybell Pit) was conducted by Deb E. Gbnima of <br />the DRMS. Colorado Division of Highways representatives Dan Tibbs and Nancy Dunsmuir were present during this inspection. <br />The pit is located approximately 1 mile west and Y. mile north of Maybell, CO and currently consists of approximately 15.2 <br />acres where approximately 15.2 acres have been disturbed. <br />While the mine entrance sign was well-placed and legible, this site lacked clearly distinguishable permit boundary markers. <br />Therefore, per Rule 3.1.12(2) the operator must clearly mark the boundaries of the affected area with monuments or other <br />markers that are clearly visible and adequate to delineate such boundaries (see Problem No. 1 and the corresponding <br />corrective action noted on page 3 of this report). <br />The large southeastern portion of the site is adjacent to Highway 40 and consists of a flatly graded area containing at least 3 <br />stockpiles of material and a topsoil berm that appears to run the extent of the southern and eastern permit boundaries (see <br />photos 1-3). There is also a section of disturbance to the north of the main active area that has been scraped relatively level <br />and contains two mounds of material at the southwestern corner of the area (see photo 4). It was explained that this portion of <br />the site is scheduled for reclamation his fall. The operator(s) plan to reapply topsoil and reseed this northern portion of the <br />permitted area. However, at the time of this inspection it appeared that the majority of the site is still active, and no reclamation <br />had yet begun. <br />In addition, the map provided to the Division for this site is inadequate. Therefore, an updated map, illustrating onlythe current <br />disturbance is required (see Problem No. 2 and the corresponding corrective action noted on page 3 of this report). <br />All other activities and completed work at the time of inspection appeared to follow the guidelines and regulations set forth by <br />the state and in the mining and reclamation plans for the pit. The sile is stable and no disturbance was observed outside of the <br />approved permit boundaries. At the time of this inspection there appeared to be no long-term (greater than 72 hours) water <br />retention. As a final note far future reference, this site is correctly positioned in the DMG online GIS. <br />Responses to this inspection report should be directed to Deb E.
