• (Page 21 • <br />MINE ID >E OR PROSPECTING ID k M-1986-102 <br />INSPECTION DATE 9/20/01 <br />INSPECTOR'S INITIALS RCO <br />This inspection was performed by the Division as part of its monitoring of 110 permits. The Division attempted to contact <br />the contact person named in the file (Dan Collins) who has also submitted the recent annual reports, but no response was <br />made to the answering maching message. There was no mining activity and no personnel present at the site during the <br />inspection. <br />The site is not mined regularly, but appears to be secured and stable. The entrance gate was locked, with the permit ID <br />sign posted there. The pad outside the portals was delineated by cable andlor berm and boundary signs were posted there. <br />The portals were all closed by locked doors, grates, or placement of structural barrier and partial earth backfill. The 4th <br />portal (numbered from either end) has a small opening through which unauthorized entry could be gained. This is a 1.5' <br />x 3' gap between the boards and grate, and should be checked by the operator and remedied, if the opening is not <br />intended. The 2nd portal from the west has a modular steel explosive magazine placed at its opening and backfilled around. <br />There are 7 portals, which appear secure at this time, but which do not necessarily exhibit final closure. <br />The bond amount includes the cos[ of all closures, and other structural removal and clean-up costs. If the operator wishes <br />to perform any final reclamation such as closures, grading, seeding of certain areas, they should be adequately documented <br />on the annual reports. If bond reduction is desired, justified by final reclamation performed, please refer to Rule 4.16. The <br />bond is 512,000, and was recalculated in July 2000. It is adequate at this time. <br />The ramp road up to the portal pad is also part of the permitted area. The operator has constructed a series of runoff <br />control ponds along the lower part of the road. The road is Bullied up to a foot deep, but the runoff ponds appear to be <br />catching all transported sediment. The operator must clean out these ponds before winter, however, since they are almost <br />full of sediment, and will have no capacity by next spring. At this time there is no problem, but the monitoring and <br />maintenance must be done before a problem develops. <br />The pad consists of the sandstone layer underlying the limestone. The grading of the pad surface appears to contain all <br />runoff. There was no debris observed outside the portals, no noxious weeds, no fuel storage, and no contamination. No <br />equipment except for the small crusher was present. The equipment was set up on and accessible by use of broken <br />limestone piles and ramps. These will be removed from the pad upon final reclamation. The site is powered by overhead <br />electric service. The supply pole has 3 non-PCB transformers. There was a small pile of overburden or soil near the <br />highwall at the west end of the pad, presumably to be used for reclamation. <br />The narrow ridges of limestone outcrop on the south edge of the pad are still present. If they are to be removed for stability <br />or safety of the site, please contact this office. <br />There were no additional items noted and no problems at this time.
