ORIGINAL - POBLIC FILE <br />(Page 2) <br />MINE ID # OR PROSPECTING ID #: ~-~ 4Rn_~ee <br />INSPECTION DATE: 9/2/nd INSPECTOROS INITIALS: ~lC- <br />OBSERVATIONS <br />The Division conducted a monitoring and construction progress inspection of <br />the site on 9/9/04. Inspected the Phase IVC construction area. <br />Approximately 210,000 square feet of liner AREA is left to be deployed, to <br />complet the Phase IVC, leach pad area, according to Mark. Clay, geomembrane <br />and drain cover were being deployed at the time of the inspection. The <br />deployed liner was walked and inspected no problem noted. Golder QA/QC <br />personnel were present at the time of the inspection over looking the <br />different areas of deployment. Once the liner deployment is completed, the <br />focus will be on completing the are under Amendment # 7 and # 8, along <br />Phase III AREA, that will be covered under TR 44.The TR was hand delivered <br />on 9/8/04.As reported in the last inspection, part of this area covering <br />38,685 square feet was approved in Amendment # 7 but was never constructed. <br />Now CC&V wants to construct the area in addition to 30,015 square feet that <br />was approved in Amendment # 8 in the same area. As reported before, the <br />total tonnage on the pad will remain at 262 million tons, as approved in <br />amendment # 8.The foot print of the pad will also be the same as was <br />approved in amendment #8,reclamtion map. Therefore, there will be no <br />additional warranty needed for the TR. The TR was requested, so there is a <br />good tracking of the pad area construction record. <br />Inspected the Phase I, II, III AND IV pregnant solution collection areas <br />and recorded the following readings from the high and Low volume solution <br />collection transducers. <br />Phase I High volume solution collection transducer # 1 was recorded at 33.6 <br />#2 was at37.2 and # 3 was at 37.0. Low volume # 1 was at.22 and # 2 was at <br />.38. Piezometer for the pond was at 37.0 FEET <br />PHASE II AND III High Volume solution collection transducer # 4 was at 39.1 <br /># 5 was at 31.3 and # 6 was at 39.3. Low volume transducer # 1 was recorded <br />at .10 and # 2 was at .57. Piezometer was at 40.3 feet. <br />Phase IV High and Low Volume solution collection transducer # 7 was <br />recorded at 28.9 # 8 was at 28.1 # 9 was at 29.1 #10 was at 29.1. Low <br />volume solution collection transducer # 1 was recorded at 12.3 and # 2 was <br />at 12.3 inches of hydrostatic head. (ALL OTHER READINGS ARE IN FEET OF <br />HYDROSTATIC HEAD UNLESS NOTED) <br />Inspected the external pond, it was holding approximately the same amount <br />of water as observed last week. While in the area, collected the following. <br />readings from the Low volume solution collection transducers for the pond. <br /># 1 was at 12.9 and # 2 was at 10.2 inches of hydrostatic head. Inspected <br />the under drain area all three pipes were discharging, no problem noted. <br />
