(Page 2) <br />MINE ID # OR PROSPECTING ID # M-1977-020 <br />INSPECTION DATE 11-13-2003 INSPECTOR'S INITIALS SSS <br />OBSERVATIONS <br />This was apre-operational inspection of the proposed Amendment No. 3 (AM-01) area to the Pit No. 1 site. <br />it was noted that several permanent man-made structures to the north and northwesf of the AM-03 area are not located on the <br />application maps. These structures include several small buildings, fences, water lines or irrigation equipment, etc. Please <br />ensure these are included on the application maps and are noted (with their owners) and addressed in Exhibit S of the <br />application. <br />The north boundary ofthe AM-02 area has a specified buffer zone and protected fence that is currently specified as the divider <br />between the final lake on the black property and the lake on the Laughery properly. This area has been mined through, though <br />the permit has not been revised to allow for this. Please ensure the AM-03 application addresses these revised mining and <br />reclamation plans and also includes authorization for removal of the fence located there.
