(Page 41 •

MINE ID rY OR PROSPECTING ID !1 M-86-061
INSPECTION DATE 1/11/00
INSPECTOR'S INITIALS RCO
The following problems IPBI and/or possible violations (PV) (and suggested corrective actions) were identified during
this inspection. The problems should be corrected by the dates given, or they will become possible violations. The
possible violations should be corrected by the dates given to reduce their severity when considered by the Mined Land
Reclamation Board. The inspector noted on the previous page should be notified of all corrective actions taken.
PROBLEMS/POSSIBLE VIOLATIONS
AND
CORRECTIVE ACTIONS
2.
3
4.
CORRECTION
UHIE
The site lacked markers which adequately delineated the permit area boundary, 2/14/00
though they are required by Rule 3/1/12. PB or PV: PB
TOPICIS): SM
CORRECTIVE ACTION: The operator must accurately locate and install all lacking _ _ _
permit boundary markers, and provide evidence to this office of such installation in
the form of photos, by the date shown at right.
The site contains an infestation of thistles, and it is not known if there exists an 3/1 /00
adequate noxious weed control plan. PB or PV: PB
TOPICISI: RV
CORRECTIVE ACTION: The operator must provide a weed control plan, and the _ _ _
commitment to timely treat the noxious weeds on the site, by the date shown at
right. (Note: the plan must be prepared by a qualified weed control professional.)
The offsite disturbance has adversely affected the stability of the soils in the gulch
and contributes to the potential for uncontrolled sedimentation leaving the site.
CORRECTIVE ACTION: The operator must install adequate sediment control
structures downslope from all unstable and unprotected areas which report to
Cottonwood Creek. These structures must be adequately installed and maintained,
at least for as long as it takes to stabilize or reclaim the upslope disturbance.
Evidence of this installation must be received at this office by the date shown at
right.
There is mining-related disturbance outside the permit area boundary, which
constitutes a possible violation.
CORRECTIVE ACTION: The matter will be heard by the Mined Land Reclamation
Board, who will determine if a violation has occurred. The Board will determine what
corrective actions are required and set [he correction dates.
4/5/00
PB or PV: PB
TOPICISI: SC ,
SP , ,
The Board will
set the date.
PB or PV: PV
TOPICISI: OD
