IV. ENFORCEMENT ACTIONS <br />The following identifies enforcement actions taken during this inspection, the required abatement and <br />date of abatement for the violation, previous violations still requiring abatement and the termination of <br />violations cited during previous inspections. <br />The following enforcement actions are still active and two have been extended (002 & 003) and two have <br />been amended (003 &004): <br />NOV CV-2000-002, for failure to provide to the Division a certificate certifying that the applicant has a <br />public liability insurance policy in place. Act 34-33-110(5), Rule 2.03.9(1) <br />NOV CV-2000-003, for failure to maintain copies of all records, reports, and information for public <br />availability at a convenient place in the area of mining. Act 34-33-122(8), Rule 5.02.4(1) <br />NOV CV-2000-004, for failure to provide the name, address, telephone number, and SSAN of the <br />applicants' resident agent.
