The problems should be corrected by the dates given, or they will become possible <br />violations. The possible violations should be corrected by the dates given to reduce their severity when considered <br />by the Mined Land Reclamation Board. The inspector noted on the previous page should be notified of all <br />corrective actions taken. <br />PROBLEMS/POSSIBLE VIOLATIONS <br />AND <br />CORRECTIVE ACTIONS <br />CORRECTION <br />DATE <br />1. During a review of the files related to this inspection, it was discovered that a copy of the OS / 28/ 04 <br />well permit for the site has not been received by the Division as specified in Stipulation 2 of <br />the permit. Groundwater has been exposed in Pod 1, which was not to have occurred <br />until a coon of a valid well permit was submitted to the Division. This is a problem related to <br />34-32-1 1 bf 1) of the Colorado Revised Statutes, for failure to comply with requirements <br />prescribed by the approved permit. <br />PB or PV: ~B_ <br />TOPIC(S): ~)~ <br />Corrective Action: Submit a coov of the valid well permit from the Office of the State <br />Engineer to the Division within 30 days of receipt of this inspection report. <br />
