(Page 2)
MINE ID # OR PROSPECTING ID #: ~-i9e~-n9e
INSPECTION DATE: ~/inrns INSPECTORS INITIALS: ~_
OBSERVATIONS
This inspection was conducted as part of the 4-year monitoring program conducted by the Division of
Minerals and Geology. Present at the inspection were Kate Pickford and Harry Posey, DMG, Greg Knell
and Jason McGraw, Robinson Brick Company, and Brad Mueller, Siloam Stone, Inc. Weather at the
time of the inspection was clear, windy, and approximately 45 degrees.
2. A sign is posted at the entrance to the site and the boundaries are marked. The site is adjacent to two
other permits. This permit is identified as the area including a portion of the pre-law disturbance at the
site; to the roadway; to the clay piles that are stockpiled relative to the Robinson Brick Company permit
)M-1977-326). A berm is located on this permit along the western edge of the site. This berm is going to
be extended to the north to connect to the topsoil stockpile located in the Bedrock Mine No. 1 permit
(M-1997-086) to control runoff from the site into the drainage located north of the site.
3. Activity at the site appears to fall within the guidelines of the permit. The current bond amount of $8,300
appears to be adequate for reclamation of the site. A partial bond calculation, however, will be
conducted in order to confirm that the amount is adequate.
I & E Contact Address
NAME: Ferd Mueller
OPERATOR: Siloam Stone
STREET: 13Gn Rudd AVP.n11P.
CITY/STATE/ZIP: Cnnnn City.
