ORIGINAL -PUBLIC FILE <br />(Page 2) <br />MINE ID # OR PROSPECTING ID #: nn_IOU~_ns3o <br />INSPECTION DATE: L21n~ine INSPECTOROS INITIALS: ~~ <br />OBSERVATIONS <br />The partial inspection was conducted while in the area, to hand deliver the <br />inspection report, DATED 11/17/04, that was returned to DMG, because the <br />listed address in the Data Base was incorrect and the site had a possible <br />problem that needed addressing by 1/27/05. Present during the partial <br />inspection was Mr. Jon Miner the operator. He stated that he would submit <br />an address change so the correct address will be entered in the Division's <br />database. Prior to driving to the pit area, I explained briefly about the <br />possible problem with the permit and the need to provide proof from the <br />County that the site is designated to accept fill material. He was also <br />informed that until such proof is submitted, no fill material could be <br />accepted. In lieu of the certification report, the operator can submit a <br />Technical Revision, to include in the backfilling of the pit, used concrete <br />and other inert fill materials that will not require a certification of <br />designations from the County and the CDOH&E. He agreed to comply with the <br />Division's request as originally identified in the 11/17/04 inspection <br />report hand delivered on this day.
