• (Pnge 3) • <br />MINE ID ~ OR PROSPECTING ID / M-77-193 <br />INSPECTION DATE 11/27/96 INSPECTOR'S INITIALS TAS <br />appear to be adequate to contain the volume of runoff generated from a :LO year - 24 hour <br />storm event. We therefore direct this matter to the attention of the Water Quality Control <br />Division. it seems reasonable and prudent for the operator to install temlarary controls or <br />complete construction of the permanent etormwater control structures as soon ae possible in <br />order to properly manage etormwater flows expected during the 1997 runoff season. Currently, <br />there dose not appear to be adequate storage capacity in the existing pondi~ and there are no <br />collection ditches or culverts in place to direct and control the flow of etormwater from <br />pond 2 to pond 3 and from pond 3 to pond 4. <br />8. The location of the proposed new haul road was inspected. It appeared ghat a significant <br />cut and fill may be necessary in order to achieve the proposed design. Mr. Tezak indicated <br />that all pertinent approvals from the local authorities have been secured and final designs <br />will be forthcoming shortly. This inspector indicated that stability of the haul road ae <br />well ae the upslope embankment may become a permitting issue. The Division will evaluate <br />this issue following review of the engineering designs. <br />9. Mr. Don Hellman, mine manager, who was present during previous inspections by the <br />Division in 1995 and 1996 was no longer with Tezak Heavy Equipment Company; Inc.
