(Page 21 MINE ID # OR PROSPECTING ID # M-1977-416 INSPECTION DATE 10/8/03 INSPECTOR'S INITIALS RCO OBSERVATIONS This inspection was performed by the Division as part of its monitoring of Hard Rock and Metals 110(2) permits, and to discuss possible changes to be requested in an upcoming technical revision. The operator was contacted about the inspection and a time was arranged to meet onsite. The operator's representatives named on page one were present during the inspection. The required permit ID sign was posted on the gate at the site (the permit area falls on both sides of the fence and gatel. An upcoming technical revision will likely include changes to the permitted area boundary. The permitted area boundary was not fully marked at the time of the inspection. This is not a problem, but whether the boundary is revised by next season or not, the operator must ensure that the boundary (as it exists at that time) is adequately marked by next field season. Markers should be durable and visible, and should at least be placed at the corners. Additional markers between corners are recommended if sighting from corner to corner is not possible. The access road from the county road to the portal area is also included in the permitted area acreage, though the road itself does not need boundary markers. The waste dump still contains the portal, steel portal shed, steel building, upland runoff diversions, and stormwater runoff control structures below the dump. The western dump has not been graded, and its slope is steeper than 2:1 presently. The eastern dump was graded in 2002 lat the time of the removal of the failed cribbingl, and its slope is 3:1 or less. The margins of the dump pad were well bermed, and a small amount of stormwater was ponded on the pad. The shop building is not secured against unauthorized entry, and the interior is becoming dilapidated. There was no debris or contamination, and no noxious weeds (though the operator should monitor for new infestationsl• There are several old structures in or near the NW part of the permit area. The old (pre-law) Carnation portal was backfilled and completely closed recently. The pre-law dump and cribbing there still exist. They have not been redisturbed by the operator and will probably remain after reclamation. The steel Butler building is not historic, and will be removed during reclamation. The main adit is closed temporarily by earthen fill at the mouth of the portal shed. This is adequate to safeguard the opening, but should be monitored to ensure it remains closed. Final closure will consist of removal of the steel shed, backfilling the adit with rubble and debris, and earthen fill in the adit. Final sloping will be no steeper than 2:1. stormwater control berms in the NE part of the permit area appear to be in functional condition and are becoming vegetated on their own. The bond amount is currently 516,236. This amount is due to be reviewed, possibly after any upcoming revision is approved. However, the present amount is estimated to be adequate at this time. No further items were observed or discussed during the inspection. No problems were observed onsite.
