~! <br />a <br /> <br />INSPECTION REPORT - PAGE 2 <br />FILE N0. <br />DATE: <br />OBSERVATIONS OF IMPORTANCE: <br />AND RECOMMENDATIONS <br />1) Technically, Hazeltine Investment Association is the operator with a <br />financial warranty of $13,000 on deposit with the State Treasurer's Office. <br />At the May 29, 1980 Board meeting, the Board approved a $14,000 financial <br />warranty and a T.R. <br />2) To date, the Board has not approved a Succession of Operator from Hazeltine <br />Investment Association to Albert Frei & Sons, Inc. <br />3) Upon review of the existing mine related disturbance, it is obvious that a <br />$14,000 financial warranty would be insufficient to reclaim the site if the <br />state was required to do so due to default. Therefore, please provide a map <br />which clearly portrays all of the existing disturbance, indicate the number <br />of acres disturbed, cubic yards of topsoil and overburden stockpiled, lake <br />shoreline grading to 3:1 as per rule 6, etcetera. Also please provide a de- <br />tailed reclamation cost breakdown for reclamation of the site at the worst <br />case i,e.; todays disturbance plus additional disturbance planned for, I <br />request that this be provided in 60 days from the date of this inspection. <br />report so that the information can be reviewed and the amount agreed upon <br />for presentation to the Board. The Succession of Operator request will be <br />RAIZ~I~IQ~MRAxR~9lR>; presented to the Board at the same meeting. <br />4) All perimeter shorelines will eventually need to be graded to 3:1 except for <br />swim beach areas which will require a 5:1 slope. <br />5) Topsoil has been stockpiled for replacement to 8-12 inches over all disturbed <br />areas. <br />6) Aularge diameter concrete pipe must be removed from the river area and broken, <br />followed by removal of re-bar if this concrete is to be used for rip-rap (see <br />August 28, 1984 inspection report for file no. 77-526 for a short discussion <br />of proper rip-rap procedures). <br />See Slides Role A 1-17 <br />(Rev. 9/83) <br />Doc.
