(Page 4) <br />MINE ID # OR PROSPECTING ID # M-1993-027 <br />INSPECTION DATE 9/29/05 <br />INSPECTOR'S INITIALS RCO <br />The following problems (PB) and/or possible violations (PV) (and suggested corrective actions) were identified during this <br />inspection. The problems should be corrected by the dates given, or they will become possible violations. The possible <br />violations should be corrected by the dates given to reduce their severity when considered by the Mined Land Reclamation <br />Board. The inspector noted on the previous page should be notified of all corrective actions taken. <br />PROBLEMS/POSSIBLE VIOLATIONS <br />AND <br />CORRECTIVE ACTIONS <br />CORRECTION <br />DATE <br />1. Some topsoil has not been stripped and is being allowed to degrade, though the 11/30/05 <br />approved plans do not allow that. PB or PV: PB <br /> TOPIC{S): TS <br />CORRECTIVE ACTION: The operator must provide a report detailing the volume of _, _, <br />stockpiled topsoil and whether it is sufficient for replacing on the disturbed area at the <br />minimum required depth. A statement committing to salvaging all topsoil from future <br />affected areas must accompany this report. These are to be received by this office by <br />the date shown at right. <br />2. Woody debris has been imported to the site, though the approved plans do not allow 11/30/05 <br />that. PB or PV: PB <br /> TOPIC(S): MP <br />CORRECTIVE ACTION: The operator must either remove the debris and provide a _, _, <br />statement and photos showing that the debris has been removed, OR request a <br />technical revision to include importing offsite generated materials to the pit. The <br />submittal for the option chosen by the operator must be received at this office by the <br />date shown at right. <br />3. The permit boundaries were not adequately marked, though that is required by Rule 11/30/05 <br />3.1.12. PB or PV: PB <br />TOPIC(S): SM <br />CORRECTIVE ACTION: The operator must mark the boundaries and send a statement _, _, <br />and photos to this office showing the installation of the markers.
