(Page 2) <br />MINE ID # OR PROSPECTING ID # M-1993-027 <br />INSPECTION DATE 9/29/05 INSPECTOR'S INITIALS RCO <br />OBSERVATIONS <br />This inspection was performed by the Division as part of its monitoring of Construction Materials 110 permits. The operator <br />was contacted about the inspection, but was not present during the inspection. The site has been recently active, but was <br />inactive at the time of the inspection. The entrance gate near the county road was locked, so access to the site for this <br />inspection was on foot. <br />The required permit ID sign was posted along the access road to the site. The permit boundary corner markers were not <br />The pit floor is level, except for a small excavated area in its center. The only equipment onsite was a Cat 988 loader in the pit. <br />There was a very small debris pile in the pit. No fuel or other hydrocarbons are stored on the site. No stockpiled pitrun or <br />gravel material was contained onsite. <br />The pit walls are vertical and approximately 8-12 ft high. No backfilling or slope shaping has been carried out. The pit daylights <br />partially on the north and on the south, but runoff still appears to be contained in the pit. <br />Above the highwall in the SW corner of the pit there was a pile of imported wood debris, measuring approximately 60' x 12' x 3' <br />review Rule 3.1. <br />requesting a technical revision. The fee for a technical revision is $188.) <br />The entire affected area was estimated to be 6 acres or less. Overall, the site is kept free of mining-related debris and <br />contaminants. No noxious weeds were observed at the site. <br />The bond is $25,000. Given the size of the disturbed area and the type of earthwork to be done, even including the removal <br />costs of the woody debris, the bond is adequate to complete reclamation. <br />No further items were observed during the inspection. Additionally the operator must submit a statement committing to salvaging all topsoil <br />from areas of this permit to be affected in the future. <br />
