(Page 2) <br />MINE ID # OR PROSPECTING ID #: M-r gRn-~aa <br />INSPECTION DATE: r r-nR-m INSPECTORS INITIALS: ~- <br />OBSERVATIONS <br />The Division conducted a monitoring and construction progress inspection of the site on 11-B-O1. Present during the <br />inspection were Mark Tidquist and Phil Barnes of CC&V. the are had received approximately 6 inches of snow the night <br />before and most of the permit area was covered with snow. Inspected the external pond, it was holding a lot of water. <br />As reported in earlier inspections, the water from the pond was being pumped on as need basis to the process plant. One <br />of the low volume solution collection sump pumps was operational, according to Phil, both pumps should be operational <br />by the end of the month. No processed solution will be placed into the pond, until the pond certification submitted to the <br />Division ON SO-245-O1 is approved and a written notification is sent to the operator, which would require, an operational <br />pump back system, PRIOR TO PLACMENT OF PROCESSED SOLUTION. Inspected the underdrain discharge area, all three <br />discharge pipes were discharging at the time of the inspection. The North underdrain discharge was being pumped into <br />the external pond. <br />Inspected the Phase II PREGNANT POND AREA, and recorded the following readings from the High and Low volume <br />solution collection system transducers. <br />LOW VOLUME SOLUTION COLLECTION TRANSDUCER PUMP # 1 WAS RECORDED AT .17 AND # 2 WAS AT .25 <br />HIGH VOLUME SOLUTION COLLECTION TRANSDUCER # 4 WAS AT 25.2, # 5 WAS AT 32.0 AND # 6 WAS AT 38.0. ALL <br />MEASURMENTS WERE IN FEET OF HEAD. <br />THE HEADER PIPE FOR THE PHASE III LEACH PAD WAS STILL BEING DIRECTLY PUMPMED INTO THE PHASE II <br />PREGANAT SOLTION POND USING A SMALL SUMP. No major problem noted. Highway 67 relocation work is complete, <br />Inspected the Load Out Bin and crusher area construction for Phase IV. Work was continuing on the filfickler wall. The <br />lined area that was to be used as staging for Phase IV construction which had been covered with a minimum of five feet <br />of drain cover for protection from vehicle traffic impacts, had a couple of temporary BUBBLE STRUCTURES PLACED ON TT <br />TO BE USED LUNCH AND EQUIPMENT STORAGE FOR EMPLOYEES.
