<br /> <br />MINE ID # OR PROSPECTING ID #M-80-244 <br />INSPECTION DATE 128-99 <br />(Page 2) <br /> <br />INSPECTOR'S INITIALS BK_ <br />The Division conducted a monitoring inspection of the phase II AND III expansion areas. <br />Present during the inspection were, Mr. Mason Estes and Mr. Terry Mandziac of CC&V. Drain <br />cover has been placed in all the lined areas of Phase II AND III expansion areas. By the end <br />of the working day, it is anticipated that all lined area that will be certified under the <br />pending certification report will be covered with drain cover material. Inspected the upper <br />100 year 24 hour surface water diversion, work was continuing, according to Mr. Mendziac, <br />concrete should be poured in sometime in the afternoon. Since it is going to be included in <br />the certification report, the work should be completed no later than Saturday or Sunday. CC&V <br />plans to submit the as built certification report to the Division no later than the 2nd of <br />November. <br />Inspected the underground remediation areas for phase three which will not be covered under <br />the certification report at this time. Mr Mendziac pointed out the planned remediation for <br />the underground workings located and the approximate location of the temporary diversion that <br />will be constructed for the area. No major problem noted. Inspected the small area south of <br />the load out bin that was lined with clay during the last inspection. It has been lined with <br />geomembrane, drain cover should be placed as soon as soon as possible. However, no ore will <br />be placed unless the operator submits an as built certification report throughout a revision <br />process.
