The problems should be corrected by the <br />dates given, or they will become possible violations. The possible violations should be <br />corrected by the dates given to reduce their severity when considered by the Mined Land <br />Reclamation Board. The inspector noted on the previous page should be notified of all <br />corrective actions taken. <br />PROHLEM3/POSSIBLE VIOLATIONS <br />AND <br />CORRECTIVE ACTIONS <br />DATE <br />1. On June 5, 1990, the Division approved the Hopemore & Leadville _ <br />Mine with four (4) conditions to the permit. The Division has not PB or PV:PB <br />received any of the information required by the conditions. TOPIC(S):ES <br />October 31, 2001 <br />CORRECTIVE ACTION: <br />The operator will need to submit the information required by the <br />Division in the June 5, 1990 letter approving the permit with <br />conditions (see attached) by October 31, 2001. As an <br />alternative, the operator can commit to in writing not to <br />dispose any additional mine tailings or other mine related <br />material in the tailings pond until all four conditions have <br />been addressed to the satisfaction of the Division. Such <br />commitment must be received by October 31, 2001. <br />2. Mining related equipment and material retrieved from underground <br />was stored outside the permit boundary at the Hopemore Shaft PB or PV:PS <br />area. TOPIC(S):OD <br />November 30, <br />CORRECTIVE ACTION: 2001 <br />The operator will need to move all mining related equipment and <br />material inside the permit area and submit proof in the form of <br />photographs by November 30, 2001. <br />3. Small trees are starting to emerge on the tailings pond PB or PV:PH <br />embankment that may jeopardize the integrity of the structure. TOPIC(S):ES <br />November 30, <br />CORRECTIVE ACTION: 2001 <br />All trees will need to be removed from the tailings pond <br />embankment and submit proof in the form of photographs by <br />November 30, 2001. <br />
