(Page 2) <br />MINE ID # OR PROSPECTING ID # M-1977-378 <br />INSPECTION DATE 8/21/07 <br />INSPECTOR'S INITIALS WHE <br />OBSERVATIONS <br />This inspection occurred as part of the Division's routine monitoring plan for permitted operations. The Sunnyside <br />Mine permit includes lands at the American Tunnel facility, Mayflower Tailings Pond No. 4, Terry Tunnel, and Lake <br />Emma area in Sunnyside Basin, totaling 148.93 acres. The Operator pursues final reclamation at all permit areas. <br />Affected lands will be reclaimed to support wildlife habitat and rangeland post-mining land use. The Division holds <br />$60,534 financial warranty. This inspection included all permit areas. Twelve photos accompany this report. <br />Backfill and grading of the collapsed Lake Emma appears to be near completion. Evidence of settling of the backfill <br />material is currently limited to isolated surface cracks located along the north side of the collapse area. The <br />Operator addresses the issue with periodic grading, occurring once in the last three years. The bottom of the <br />backfilled basin has been graded to provide positive drainage to the south. Evidence of active settling of the backfill <br />material was not observed. Evidence of erosion or invasion by undesirable weed species was not observed. <br />The mine bench at the Terry Tunnel had been recently scarified in preparation for another application of the <br />reclamation seed mix. Previous efforts at revegetation for the mine bench had exhibited unsatisfactory results. <br />Reclaimed areas appeared stable, evidence of slumping, settling or erosion were not observed. Weed control <br />efforts have eradicated a previously established infestation of Canada thistle. A minor occurrence of toad flax was <br />present. Upland drainage and drainage from a spring in the area of the backfilled portal is conveyed around the <br />mine bench by open ditch. Upland drainage and the drainage nearby the backfilled portal appear to be associated <br />with a series of springs located upslope of the mine bench and trending in a line. <br />The Operator briefly discussed options for terminating monitoring at station EC-1. Station EC-1 monitors surface <br />waters from Eureka Creek downstream of Sunnyside Basin and immediately upstream of the Terry Tunnel. The <br />purpose of EC-1 was to monitor impacts of surface mining activities at the Sunnyside Pit and Washington Pit, <br />located in Sunnyside Basin and nearby Lake Emma. Surface mining at the Sunnyside and Washington Pit was <br />approved April 27, 1988, through AM-03. On December 17, 1996, the Division approved TR-20, which terminated <br />slope monitoring of the talus slopes above the Sunnyside and Washington Pits. On February 1, 1998, the Division <br />approved SR-02, which released the Sunnyside Pit and Washington Pit from the permit for successful completion of <br />reclamation. Therefore, termination of monitoring at EC-1 appears appropriate. Should the Operator desire to <br />discontinue monitoring at EC-1, please submit such request through the Technical Revision process. <br />Backfilled and graded areas at the American Tunnel appeared stable, no evidence of slumping, settling or <br />excessive erosion was observed. Vegetative cover was establishing. Survey lath and monuments have been <br />installed by BLM to delineate federal properties administered by the BLM. Drainage from the American Tunnel <br />portal is conveyed through the backfill by a 24 inch diameterABS corrugated culvert with steel grate closure. The <br />mine drainage is conveyed to Cement Creek by an open ditch located along the north end of the mine bench. <br />One excavator was working on top of the Mayflower Tailings Pond No. 4, applying the final 2 feet of overburden and <br />topsoil cover. The top of Pond 4 had been previously reclaimed but a small cell was subsequently reopened to <br />allow placement of sludge from the final reclamation of the treatment ponds at the American Tunnel.
