~.r • <br />%-.- <br />(Page 2) <br />MINE ID # OR PROSPECTING ID #: M-A0.244 <br />r~ <br />L <br />INSPECTION DATE: h-10.00 INSPECTOR'S INITIALS: ~_ <br />OBSERVATIONS <br />The Division conducted a monitoring and construction progress inspection of the area on Fr15-00. Present during the <br />inspection was, Scott Lewis of CC&V. The inspection began in the Phase III area. Both clay and geomebrane liners were <br />being deployed at the time of the inspection. Approximately 3l4 of the phase III ore storage area had already been <br />deployed with geomembarane liner. The deployed liner was walked and inspected, no major problem noted with the <br />exception of Panel # 39 which had some folding in the liner. Operator stated that during drain cover placement, most of the <br />fold will be removed and noted the panel #. Drain cover was being placed on the already clay and geomembrane lined area. <br />Golder QA/QC were present overseeing the operation as required in the approved construction specification. Due to dry <br />spring, water was being applied on to the clay liner using water trucks to make sure the optimum moisture is achieved prior <br />to compaction. According to the operator, the Phase III area construction should end with in two weeks. <br />Inspected the Phase II pregnant solution area and collected the following readings from the high volume and low volume <br />solution collection system transducers. <br />Low Volume pump #1 read .42 feet of head <br />#2 read .20 feet of head <br />High Volume pump #4 read 20.0 feet of head <br />#5 read 25.8 feet of head <br />#6 read 24.2 feet of head <br />The Phase II toe dam raised area has been covered with geomembrane liner. Apparently the liner installers decided to <br />deploy the liner as approved in the Amendment so no change to the approved design was necessary. A portion of the <br />deployed liner had some air bubbles, Bob Nickels of Golder was notified to deploy drain cover under the same liner <br />deployment temperature to remove the bubble. He agreed and stated that it was their intention to do just that. Observed <br />the removal and replacement of the low volume solution collection transducers for Phase II. The transducers did not show <br />any wear such as scaling, etc....
