(Page 3) <br />MINE ID # OR PROSPECTING ID # M-2001-054 <br />INSPECTION DATE 3/3/04 <br />INSPECTOR'S INITIALS RCO <br />The operator should include the statement that is requested in the paragraph above, acknowledgement of the requirement that <br />construction and maintenance of The onsite portion of the new road will include stable cut slopes and drainage control. <br />Preventable adverse impacts to offsite areas (outside the permit boundary), due to sediment transport, erosion from <br />uncontrolled runoff, sloughing material from overly steep slopes, etc., may be a possible violation to the DMG permit and is to <br />be avoided. Offsite impacts due to uncontrollable events such as natural movement of the Bridalveil Creek channel, for <br />example, should be anticipated but might not be subject to enforcment action. <br />The operator should further note that, upon abandonment of the first road, this office will expect reclamation of the onsite <br />portion of the road to commence. This will include grading antl revegetation. It is assumed that the CDOT permit also contains <br />closure and stabilization requirements for the area in the right-of-way affected by the first road. This office assumes thatthose <br />requirements will be inspected by that agency. <br />No further items were observed during the inspection.
