• (Page 2)
MINE ID # OR PROSPECTING ID # M-94-117
INSPECTION DATE 07/15/98 INSPECTOR'S INITIALS CLK
OBSERVATIONS
This inspection of this 110(2) site was conducted at the request of the Operator, in order
to va~rify the existence of undisturbed portions of the site and their location by the
Division. ITEC intends to ask for a partial release of undisturbed acreage in the future, and
to rep-set the boundaries of the permit area which were never clearly defined during the 1995
submittal of this permit application. The Operator will submit a new map, legal description,
and proof of right-to-enter the new areas specified.
The area south and east of the pond has not been disturbed; cover includes 20' trees, thick
grass; and wildflowers, and gradual slopes which have clearly not been excavated. The area
extending northeast of the bond toward the road has also been left largely undisturbed.
Shou:Ld a TR be submitted within the next 60 days, another inspection to verify the lack of
disturbance at the site will not be necessary.
The Operator was told by the Division that if the new permit boundaries include the junk
piles and boneyard" which belong to the landowner, that disposal of these items will have
to ba included in the reclamation bond. ITEC is advised to either make sure these materials
have been removed prior to the submittal of a technical revision to amend the boundaries, or
to be prepared to submit additional bond.
I & E Contact Address
NAME Maxine Stewart
OPERATOR ITEC Environmental Colo.
