• (Page 2) • <br />MINE ID # OR PROSPECTING ID #M-80-244 <br />INSPECTION DATE 11-12-99 <br />INSPECTOR'S INITIALS BK <br />The Division conducted a joint inspection of the site with a representative from Teller <br />County Government. The main purpose of the inspection was to physically visit the two Cyanide <br />spill outside of the lined areas.Present during the inspection were, Mr Scott Lewis of CC&V <br />and Miss Jane Mannon of Teller County. The physical inspection began around the second spill <br />area of October 31, 1999, where Mr Lewis gave a brief explanation of what took place and the <br />efforts the operator implemented once the spill was reported. He also pointed out the failed <br />reducer pipe which was located in the vicinity. The HDPE pipe has been replaced with steel <br />pipes and fittings. He also explained the plans that the operator has implemented and plans <br />to implement to avoid such failures in the future.Continued the inspection to the first spill <br />area of October 13, 1999. Again, Mr. Lewis briefed Miss Mannon as to the cause of the spill <br />outside of the lined areas. He explained, in addition to replacing the hdpe pipes with steel. <br />The operator is also in the process of moving most of the Header pipes up on to the leach <br />pad. Even if un foreseen failure takes place, the solution will be captured with in the leach <br />pad. While in the area, the following readings were taken from the high and low volume <br />collection pump systems. <br />HIGH VOLUME PUMPS #4 READ 18.1 FEET OF HEAD <br />#5 READ 19.5 FEET OF HEAD <br />#6 READ 23.5 FEET OF HEAD <br />LOW VOLUME PUMPS #1 READ .43 FEET OF HEAD <br />#2 READ .19 FEET OF HEAD <br />Mr Lewis also pointed out to a survey marker that was on top of the pregnant solution area <br />which shows the areas where CC&V plans to install piezometers to allow the physical <br />measurement of the actual solution level with out totally relying on the trancducers.He sated <br />that they plan to place this piezometer as soon the actual surveying is completed and <br />verified by the operator.Mr Lewis was informed of the extreme care that must be taken during <br />the drilling operation and that the Division would like to be notified so we can observe the <br />drilling operation.COntinued the inspection to the PHASE II and III certified areas. The <br />North side of the Phase II construction area, ore and leach lines have been in place and <br />solution was being applied while inspecting the area. No major problem noted. Operator was <br />granted to apply solution prior to approval of the Certification report. Approval for the <br />placement of leached ore was garnted during the Amendment #7 approval on January 24, 1999. <br />Inspected the uncertified portion of phase III expansion areas. Backfill and grading work was <br />continuing. Inspected the main pit area, it was active at the time of the inspection no <br />problem noted. Continued the inspection to the Arequa Waste ( overburden) storage areas. Two <br />lifts have been top soiled and trees planted on the slopes. Mr. Lewis briefly explained the <br />sequence of reclamation in the area. He stated that seeding should take place as soon as <br />possible. Slopes appeared stable. Continued the inspection to the North Cresson area of the <br />Phase II expansion. Active mining was taking place at the time of the inspection. <br />
