The location in <br /> the OMLR application must be indicated in the portion of the submittal addressing Teller <br /> County requirements. Such references shall be specific and facilitate location of <br /> information by reviewers. <br /> 5.1 Application form (completed) - including the following <br /> 5.1.1 Applicant. Source of the applicant's legal rights to enter and to mine on <br /> the land affected by the PUD; owner(s) of record of affected land and <br /> owners of substance to be mined, and address of the general office and <br /> the local address or addresses of the applicant. List of all adjoining <br /> property owners. <br /> 5.1.2 Office of Mined Land Reclamation application. A copy of the application <br /> including any amendments and supplemental information provided after <br /> initial submittal (complete with all pertinent maps, drawings, and charts) <br /> made to the Colorado Department of Natural Resources to comply with <br /> the Colorado Mined Land Reclamation Act (CMLRA) unless the Teller <br /> County Planning Department agrees that sufficient information to consider <br /> a mining application is otherwise supplied by the applicant. <br /> 5.2 Character of development - including the following <br /> 5.2.1 Building Design- Information. A plan showing locations of any new or <br /> expanded buildings, subject to Teller County building permit requirements, <br /> that are visible from beyond the boundaries of the CCMOD from public <br /> roads and showing the elevations with identification of the materials <br /> proposed for external construction, including the color of materials, and a <br /> description of the measures proposed to reduce contrast for buildings, or <br /> otherwise make the building complimentary to the area. Use of colors that <br /> approximate earth and vegetation tones to the degree feasible and <br /> effective will satisfy this requirement. Expanded buildings are those <br /> buildings that are proposed to be increased in exterior dimensions by <br /> more than 25% of the dimensions as approved in the original PUD. Any <br /> expansions of less than 25% shall be in keeping with the existing building. <br /> 5.2.2 PUD Plan. The following information may be referenced from the OMLR <br /> permit application. The purpose of requiring this information is to provide <br /> the County an understanding of the proposed operation for which a PUD <br /> has been requested. <br /> a. Complete legal description of the permit area and pertinent mineral rights <br /> supplemented by geographic descriptions of the location of separate <br /> permit activity areas, haul roads, utilities, limit of mining district, <br /> improvements, and major equipment within the permit area. A reasonably <br /> accurate map may be submitted to satisfy this requirement. <br /> b. Description of the overall operation to include all relevant aspects <br /> including, but not limited to, site preparation, vegetation and soil removal, <br /> mining excavation methods, crushing and screening activities, milling, <br /> processing, tailing management, overburden management, ore <br /> CCMOD - Effective May 25, 2000 6 <br />
