(Page 2)
MINE ID # OR PROSPECTING ID #: M-t eex-n3a
INSPECTION DATE: 9/7f,/n2 INSPECTOR'S INITIALS: KAP
OBSERVATIONS
1. This inspection was conducted as part of the Division of Minerals and Geology's monitoring program. Present a[ the
inspection were Kate Pickford, DMG and )oe Docheff, Mountain Park Concrete, Inc. Weather at the time of the inspection
was partly cloudy, calm, and approximately 50 degrees.
2. A sign was posted at the entrance to the site and the boundaries were marked. According to Mr. Docheff, the pi[ was
formerly a county borrow pit. The current operator is still working on phase #1 of the project. The rear portion of phase #1
has been stripped of vegetation and is ready to be mined. The areas in phases #2 and #3 have not been disturbed. A topsoil
stockpile is present in the center of the phase #1 area. According to Mr. Docheff, the pile has been seeded twice, but the grass
did not become established. Mr. Docheff further advised that a representative from the Soil Conservation Service (SCS)
inspected the site and was not concerned about the vegetation on the topsoil stockpile. The SCS representative indicated that
the vegetation on the pile would help establish grasses and that if Leaf Spurge and Canada Thistle were absent, then the SCS
had no issues with the vegetation at the site. There was no Leafy Spurge nor Canada Thistle at the site, and grasses are
beginning [o establish themselves on the topsoil stockpile. Runoff at the site appears [o be contained within the permit
boundaries. Noxious weeds were not present at the site.
3. All activities and completed work at the time of inspection appeared to be following the guidelines set forth in the mining
plan for the pit. All operations and conditions at the time of the inspection fall within regulations. A bond recalculation will be
conducted as a portion of this inspection in order [o determine whether the current bond is adequate to reclaim the site.
I & E Contact Address ^ OTHER: Carl Mount, DMG
NAME: M. loe Docheff III
OPERATOR: Mountain Park Concrete.
