DMG has informed me that subsidence monitoring <br />requirements were dropped in 1988. DMG will require the mine <br />plan be revised to reflect the status of the current <br />subsidence monitoring. <br />Pond Inspections: The last quarterly pond inspection for ponds <br />T-1, T-2 and Long was conducted on 06/15/94. The last <br />quarterly inspection for ponds P-3, P-5 and P-9 was conducted <br />on 06/21/94. - <br />Certifications: <br />Impoundments--The certifications <br />of the mine impoundments was done <br />dates: <br />Long Pond 07/14/93 <br />P-9 Pond 11/25/91 <br />P-5 Pond 02/06/84 <br />P-3 Pond 02/06/84 <br />P-2 Pond 02/06/84 <br />P-1 Pond 02/06/84 <br />Truck Dump Pond 02/06/84 <br />Treatment Pond 1 02/23/89 <br />Treatment Pond 2 02/23/89 <br />of the construction <br />on the following <br />Roads--Haul road construction was certified on <br />08/01/84. <br />Coal Mine Waste--The reclamation of the refuse pile <br />was certified on 10/16/86. <br />Public Records: The location for the mine plan and other <br />documents for this site are kept at the Garfield County <br />Courthouse. Although most of the information pertain mg to <br />the mine is stored there, a complete record was not available. <br />Inspection and bond release documents were not on file at the <br />courthouse. All public documents relating to the mine site <br />must be made available and are required to be kept with the <br />mine plan. DMG should review the file and insure that the <br />company place all required document at the courthouse. <br />FIELD REVIEW <br />Signs and Markers: The mine identifications signs were posted <br />at the entrance to the mine and the haul road to the truck <br />dump. <br />5 <br />
