'MINE ID # OR PROSPECTING ID # M-80-244
.INSPECTION DATE 9-16-99
(Page 2)

INSPECTOR'S INITIALS BK
OBSERVATIONS
The Division conducted a monitoring and progress of the PHASE II LINER EXPANSION AND PHASE
III construction areas on 9-16-99. The main purpose of the inspection was to physically
measure the solution level in the Phase II pregnant ponds using an Electronic meter. Present
during the inspection were, Mason Estes, Terry Mendziac and Scott Lewis of CC&V. The
inspection began at the Phase II pregnant pond area. The following readings were taken from
the transducers. Pump # 4 read 30.4 feet of head, PUMP # 5 read 31.3 feet of head and pump
#5 read 35.3 feet of head. Using an E meter, the following readings were measured from pumps
#4 and #5. Pump #4 read 25.1 feet from top of the caisson to the static solution elevation
and Pump #5 measured 24.4 feet. Pump #6 was not measured due to different set up. Based on
the two measurements, it appears the static solution elevation to be with in plus or minus
5 inches. The Following readings were taken from the low volume collection pumps Pump #1
read .42 feet of head and pump #2 read .26 feet of head. Flow rate was measured to be 16
G.P.M.
Inspected the Beacon Hill area of PHASE III, back fill and grading was continuing. Inspected
the Phase II expansion area. Appx. 1500 feet of the area has been lined. It was walked and
inspected, no major problem noted. Inspected the upper Squaw Gulch haul road diversion area,
it appears to be functioning as designed. An additional sump along the North side of the haul
road has been constructed for added sediment control. Inspected the North Cresson of the
phase III expansion area tree removal was continuing. No major problem noted.
I & E Contact Address cc:James Stevens
^ CE
NAME Scott Lewis ^ BL
OPERATOR CC&V ^ FS
STREET P.O.BOX 191 ^ HW
CITY/STATE/ZIP Victor Colorado 80860 ^ HMWMD (CH)
^ SE
WQCD (CH)
^ OTHER
