• (Page 3) • <br /> <br />NINE ID # OR PROSPECTING ID # M-80-244 <br />INSPECTION DATE_9-10-99 <br />INSPECTOR'S INITIALS BK <br />The Division conducted a monitoring inspection of the phase II expansion area on 9-10- <br />99.Present during the inspection were, Phil Barnes, Scott Lewis, Terry Mendziac of CC&V and <br />James Johnson of Golder and Associates. In anticipation of liner deployment, APPX. 600 FEET <br />OF THE EXPANSION AREA ORIENTING east to west has been compacted and appears to be ready for <br />geomembrane deployment. The entire lined clay lined area was walked and inspected and no <br />major problem was noted. The lined area appeared smooth no large protruding rocks were noted. <br />According to the operators representative, in the area approved by the QA/QC personnel on <br />site, liner deployment could start as soon as possible. Inspected the textured rolled loner <br />that was placed along the North side of the clay lined area of Phase II expansion, according <br />to Mr. Jim Johnson of Golder and associates, all the textured liner met the approved <br />specification. <br />Inspected the high volume and low volume solution collection areas. No solution was ponded <br />on the surface of the pregnant solution collection area. It appears the pumping of the <br />solution to the phase I area appeared to have worked. However, according to the operator, <br />they still do not know what caused the ponding in the first place. The Division requested <br />that once the operator surmises the cause of the ponding, to forward the report as requested <br />in the inspection report of 9-2-99. The following readings were taken from the high volume <br />collection pumps. <br />PUMP #4 READ 38.8 FEET OF HEAD <br />PUMP #6 READ 36.8 FEET OF HEAD <br />PUMP $6 READ 40.4 FEET OF HEAD <br />LOW VOLUME COLLECTION PUMP #1 READ .42 FEET OF HEAD <br />LOW VOLUME COLLECTION PUMP #2 READ .26FEET OF HEAD <br />FLOW RATE WAS 18.5 G.P.M. <br />An emergency generator was in place in addition to the already existing emergency generator. <br />The following readings were recorded from the high volume solution collection sumps for Phase <br />I. THE PROBES FOR PUMPS 1 AND 3 WERE NOT FUNCTIONING, PUMP #3 READ 46 7 FEET OF HEAD. <br />According to Mr.Lewis, new probes have been ordered for pumps 1 and 2. <br />Inspected the under drain area, both 24 inch pipes and the 4' pipes were discharging. No <br />samples were collected. Inspected the Iron Clad Globe hill area. Mr Lewis pointed out where <br />they plan the topsoil pile located North West of the pregnant pond area. <br />I & E Contact Address <br />NAME MR.
