The problems should be corrected by the <br />dates given, or they will become possible violations. The possible violations should be <br />corrected by the dates given to reduce their severity when considered by the Mined Land <br />Reclamation Board. The inspector noted on the previous page should be notified of all <br />corrective actions taken. <br />PROBLSNS/P033IHLB VIOLATIONS <br />AND <br />CORRBCTIVB ACTIONS <br />DATE <br />PROBLEM: <br />The Jesse Lee permit was approved for 9.8 acres in size, but PB or PV:PV <br />data collected by the Division indicate that 3.34 acres of TOPIC (S):OD <br />mining occurred outside of the permit boundary. This is a November 30, <br />possible violation of 34-32.5-116 (4)(I)or 34-32.5-109(1]. 2006 <br />CORRECTIVE ACTION: <br />Submit a complete 112 Conversion Application by July 11, 2006 <br />and have the application approved by October 30, 2006 (or <br />November 30, 2006 if comments are received). <br />
