(Page 2)

MINE ID # OR PROSPECTING ID #: M-7007-Ot S
INSPECTION DATE: fi/t8/nt INSPECTOR'S INITIALS: ICC
OBSERVATIONS
This inspection was conducted in order to check compliance of the operation with the requirements of the Act, Rules, and the
conditions of the permit. The pit was inactive at the time of the inspection; in (act, mining had not yet begun at the site, which
was permitted on 5/18/01. The permit holder, Las Animas County, was represented by Phil Dorenkamp during the inspection.
Although notice of the application for a permit was posted at the entrance to the access road, there was no ID sign posted for
the operation as required by Rule 3.1.12(1). This is a compliance problem that will be included on Page 3 of this report.
At the location of the pit, Mr. Dorenkamp identified certain of the markers for the boundaries of the permit area. The permit
area appeared to be located as indicated in the permit application. Since mining had not yet been initiated at the site, the
boundaries of the permit were not walked.
There is some old mining related disturbance within the permit area. According to Mr. Dorenkamp, the landowner insisted
that this area be included in the County's permit area. It will apparently be mined thru by the County during their operations at
the site.
CERTIFIED MAIL NO.l~o~ Ilo'ID ooo~ 0~0 ~5~8
RETURN RECEIPT REQUESTED
I & E Contact Address
NAME: Mr. Phil Dorenkamp, Road and Bridge
OPERATOR: Las Animas County
STREET: 2000 N.
