• (Page 2) • MINE ID rr OR PROSPECTING ID N M-1978-039 HR INSPECTION DATE 12/18/01 INSPECTOR'S INITIALS RCO OBSERVATIONS This inspection was performed by the Division as part of its monitoring of Hard Rock and Metal 110 permits. The operator was contacted but was not present during the inspection. The site was not active at the time of the inspection. The last inspection by the Division was on 7/10/97, conducted during the transfer of the permit to this current operator. Several items were noted in that report for the operator to follow up on, either to update the records or perform on the site. Little, if any, action has occurred to address those inadequacies. These will be noted in the paragraphs below. The locked gate has a permit ID sign from the prior operator, but not from IUC. The permit boundary markers have not been installed as required either. The lack of these items is noted as a problem in this report. Please see the last page for the correction date. In 1998 an attempt was made by the operator to update the maps, though that endeavor was never completed. The file therefore still references the earier 1979 map, which is likewise not complete or correct. Annual reports filed by the operator subsequent to 1998 note that no changes have occurred which would necessitate revising the maps, though on- the-ground features have changed enough to warrant a current map. The lack of providing a complete, correct, and current map is noted in this report as a problem. Please see the last page for the correction date. There is a trash pit on the dump pad, as well as what appears to be a small ore stockpile. The overhead power supply is not configured as the map depicts it. The shop building is locked, but is not as large as the map depicts it. The topsoil areas and the "future development area" do not exist as depicted. The question of the dump area was raised in 1998, but not answered: Does the dump boundary on the map depict the upper edge of the pad or the toe of the dumpl If the upper edge of the pad is depicted, then the toe of the dump is north of and outside the boundary line. The portal was secured against unauthorized entry. No contaminants were observed onsite. The bond will be reviewed in the near future, and the operator will be informed about any increase. No reclamation grading, or revegetation has occurred. Halogeton grows on much of the disturbed area. The operator must ensure that it does not infest the topsoil stockpiles set aside for reclamation. There were no additional items observed. The operator must respond with proof of correcting the problems by the dates shown on the last page.
