The problems should be corrected by the dates given, or they will become possible violations. The possible violations should be corrected by the dates given to reduce their severity when considered by the Mined Land Reclamation Board. The inspector noted on the previous page should be notified of all corrective actions taken. PROBLEMS/POSSIBLE VIOLATIONS AND CORRECTIVE ACTIONS CORRECTION DATE Permit boundary markers were not observed, though they are required pursuant to Rule 10/31/06 3.1.12. PB or PV: PB TOPIC(S): SM CORRECTIVE ACTION: The operator must install markers adequate to delineate the _, _, permit boundary. Evidence of such installation must be received by this office by the date shown at right. 2. There is inadequate stormwater control at the site, with no evidence of a SWMP plan. 11/30/06 PB or PV: PB CORRECTIVE ACTION: The operator must provide evidence of contact with the TOPIC(S): SP CDPHE/WOCD (address below) regarding the need for a stormwater permit and/or the _, _, installation of adequate control structures. Evidence of such contact and plans for constructing any required structure, must be received by this office by the date shown at right. 3. The operator is not providing accurate, complete, or adequate information, especially in 12/31/06 the annual reports.
