(Page 2) <br />MINE ID # OR PROSPECTING ID # M-78-314 <br />INSPECTION DATE 6/24/98 INSPECTOR'S INITIALS CLK <br />OBSERVATIONS <br />The following observations were made as part of a routine inspection of this 9.9 acre 110 <br />gravel pit. <br />1. No sign was in place with the operator's name and the DMG permit number. (PB 1) <br />2. Fencing enclosed one half of the site; no boundary markers were in place on the other <br />half. (PB1) <br />3. Approximately 8.7 acres have been disturbed at the site at this time.4.7 acres are on the <br />east side of the road, where the Operator has excavation and screening operations in place; <br />4 acres are on the west side of the road, and are being used by the County. Approximately <br />half an acre is taken up by the road dividing the two areas. None of this is apparent on any <br />of the maps in this permit file (PB 2). <br />4. Topsoil has been salvaged along the excavation rims on both sides of the site. A pile <br />approximately 630' long, 6.5' high, and 30'wide is stored (4900 CY) above a 917' long cut <br />that varies between 1H:1V and 2H:1V slopes on the Operator's half of the site. 580' of <br />topsoil is stored along the western side of the County area, and 710' of topsoil along the <br />east side --- both stockpiles varying in height from 5' to 7.5', with 5' of topsoil stored <br />along the 510' south side, for a total of 8680CY. There is a possible topsoil deficit at the <br />site of 480 CY, if the topsoil is spread 1' thick, as is specified in the permit. <br />5. Very little vegetation is present other than weeds.
