(Page 2) <br />MINE ID # OR PROSPECTING ID #: ~~-~~v-> r8 <br />INSPECTION DATE: v-4-n~ INSPECTOR=S INITIALS: ~_ <br />OBSERVATIONS <br />This was aPre-Operation inspection To determine whether there are any field issues that need to be considered <br />regarding D. G. Huskin Construction's application for a Regular (112c) permit for their proposed Velarde Pit. <br />Consultant to D. G. Huskin. Joe Gaaliano, accompanied me. <br />This site is composed of short crass gently rolling rangeland and a low northeast-southwest trending ridge with <br />iunipers at the higher elevations. The permit area is dissected west of and parallel to the ridge by a large <br />vertical sided, deep (15-20 feet) arroyo. which appears to have a large watershed. The arroyo is not in the <br />proposed affected land area but gets close (50-100 feet) in at least two locations. The arroyo is shown to be an <br />intermittent stream on the Exhibit C Maps. One of the existing ranch roads (trails) that accesses Phase 1 crosses <br />the arroyo. The crossing consists of road cuts on both sides of the arroyo down to the streambed level. There <br />was no flow in the stream at the time of the inspection. The arroyo would not be easily crossed by haul trucks <br />without significant lessening of the ingress and egress grades. Mr. Gaaliano stated that a culvert crossing would <br />be installed. The crossing plans will have to be included in the application. The streambed enters the Huertano <br />River approximately'/< mile downstream of the north permit boundary and the proposed Phase 1. <br />According to Mr. Gagliano, the two access/haul roads (existing ranch roadsltrails) will be substantially <br />upgraded by D. G. huskin. They are not shown to be affected land on the mining plan map but should be. The <br />addition of these roads to affected land will constitute an amendment to the application. (see Rules 1.8 and <br />1.6.6 ) <br />The southern access road crosses property of William J. Agnes Trustee. If the road is not relocated in the <br />application around Mr. Agnes' property, he must be shown as an owner of the surtace of affected land on <br />image 2 item 7 of the application and be notified of the application in accordance with Rule 1.6.1(e)(i). <br />A Powei line crosses the permit area and the proposed Phase 1 area. The power line is not shown on the Exhibit <br />C maps or identified in Exhibit B. It is also not addressed in Exhibit S. <br />The proposed affected land was marked with steel t-posts. The southeast corner of Phase 2 was marked to be <br />within 50-75 feet of property belonging to Joe L. and Luciano Benavides: whereas, the minincl plan map shows <br />that there will be a 200 feet setback from adjacent property owners.
