• (Page 2) • <br />MINE ID # OR PROSPECTING ID # M-93-059 <br />INSPECTION DATE 12/10/98 INSPECTOR'S INITIALS AJW <br />OBSERVATIONS <br />This inspection was conducted at the request of the operator to discuss the possible closure <br />and reclamation of the mine. The site is located approximately 3 miles west of Holly, <br />Colorado and is situated along the Arkansas River flood plain in between U.S. Hwy 50 and the <br />Arkansas River. The mine is permitted by Mid-Western Farms and operated by Holly Rock, LLC. <br />Fresent during the meeting and inspection were George Temple and Don Begge of Mid-West Farms <br />and Mike Cure and Dan Grasser of Holly Rock, LLC. <br />7'he operator indicated that mining operations have ceased as of this inspection. date, <br />however, material is still being shipped out of the extensive product stockpiles. There is <br />currently an amendment on file with the DMG that will modify the reclamtion plan and slightly <br />change the boundaries. The most significant change will be to leave the crushing, screening <br />and loadout area as an industrial site once mining is completed. If the mine were to have <br />kept operating, it also would have changed the reclamation bond by increasing it <br />approximately $200,000 for a worst case disturbance. However, the current confuguration of <br />the mine is such that the current bond is nearly adequate to cover any reclamation liability. <br />Since mining is going to be discontinued, the proposed bond in the amendment with be re- <br />calculated to address the current disturbance which will likely increase it by approximately <br />$50,000.00. The decision date for the amendment is 2/7/99 and the final reclamation <br />liability amount will be included in that approval. <br />For the time being, the operator is attempting to market the equipment and lease to another <br />operator. If this does not occur by April, 1999 an auction will be held and reclamtion will <br />commence on the site. If the mine is sold, the new operator will be responsible for posting <br />the "worst case" reclamation bond prior to beginning operations. <br />The current mining configuration has created an approximately 17 acre lake just south of the <br />overburden sand pile. The majority of the overburden pile should be used to backfill this <br />lake and any zemaining sand will be able to fit into the westside lake. If a new operator <br />does not take over the site and reclamtion commences in April, the final topography of the <br />reclaimed site will likely have one small lake on the western side of phase I and possibly <br />a small lake along the south-southeast side of phase I. These two lakes combined will be <br />much smaller than the size of the projected lake if the site were fully mined. The one <br />precaution that will need to be followed when backfilling the lake is to make sure the <br />sideslopes meet the requirements of 6H:1V along the shorelines below the projected high water <br />point. <br />This concluded the inspection. <br />I & E Contact Address <br />NAME George Temple <br />OPERATOR Holly Rock, LLC <br />STREET P.O.
