<br />MINE ID # OR PROSPECTING ID # M-77-436 <br />INSPECTION DATE_August 28, 1998 <br />INSPECTOR'S INITIALS TAS <br />The following problems (PB) and/or possible violations (PV) (and suggested corrective <br />actions) were identified during this inspection. The problems should be corrected by the <br />dates given, or they will become possible violations. The possible violations should be <br />corrected by the dates given to reduce their severity when considered by the Mined Land <br />Reclamation Board. The inspector noted on the previous page should be notified of all <br />corrective actions taken. <br />PROBLEMS/POSSIBLE VIOLATIONS <br />AND <br />CORRECTIVE ACTIONS <br />CORRECTION <br />DATE <br />1. Five (5) significant portions of the 1,100 linear feet of <br />reclaimed east pit slope exhibit severe gully erosion and <br />are not stable. <br />CORRECTIVE MEASURES <br />Repair the existing gully erosion and stabilize the 1,100 <br />linear feet of east pit slope prior to financial warranty <br />release. <br />2. The operator is not in compliance with the mine plan as <br />the pit is being dewatered; dewatering is not currently <br />allowed. <br />CORRECTIVE MEASURES <br />The operator will either cease any further pit dewatering <br />OR submit an application for a permit modification to <br />allow pit dewatering and dry mining practices. If an <br />application for pit dewatering is submitted, this must <br />be in the form of an amendment to the permit and the <br />amendment (including posting of adequate financial warranty <br />if any additional warranty is required) must be completed <br />within 120 days of submittal date.
