The problems should be corrected by the dates given, or they will become possible violations. The possible violations should be corrected by the dates given to reduce their severity when considered by the Mined Land Reclamation Board. The inspector noted on the previous page should be notified of all corrective actions taken. PROBLEMS/POSSIBLE VIOLATIONS AND CORRECTIVE ACTIONS DATE 1. The operator failed to first obtain a reclamation permit prior _ to engaging in a new operation in accordance with 34-32.5-109(1) PB or PV:PV of the Colorado Land Reclamation Act for the Extraction of TOPIC (S):SM Construction Materials or the operator failed to protect areas _, _, _ outside of the affected land from slides or damage during the mining and reclamation operations in accordance with 34-32.5- 11694)(I) of the same Act. CORRECTIVE ACTION: The operator will need to submit a new application to replace M- 81-303 that revises the total affected area to incorporate all present and future disturbances that have occurred and/or will occur as a result of this operation and provide for reclamation of the additional disturbance in accordance with the Act. A complete 112 application must be submitted by December 15, 1998. 2. A mine sign was posted at the mine entrance, but with a _ different operator than Pine Ridge Development Company. PB or PV:PB TOPIC (S):AR CORRECTIVE ACTION: _, _, _ If Pine Ridge Development Company is no longer a valid company, the Pine Ridge Permit will need to be transferred. Anew Bond and Financial Warranty will also need to be submitted. The Division will require the permit to be transferred and bond submitted by December 1, 1998 or a letter stating that Pine Ridge Development Company is the operator.
