(Page 2) <br />MZNE ID 'k OR PROSPECTING ID ',{ M-79-214HA <br />INSPECTION DATE 10/31/96 INSPECTOR'S INITIALS GSC <br />OBSERVATIONS <br />This inspection was conducted at the request of the landowners and the operator to review <br />corrective actions and maintenance items outlined in previous inspections which still need <br />co be addressed. In attendance were: Gary Curtiss and Mike Long of the Division, Steve <br />Brown, attorney for the Division from the Office of the Attorney General, Mike Sheahan and <br />John Hickman of Western Mobile (Rocky Mountain Rock Products), Dean Massey, attorney for the <br />operator, Henry Hall, the landowner and Ric Fanyo, attorney for the landowner. <br />The corrective actions and maintenance items mentioned above were previously identified in <br />inspections conducted on 3/2/95 and 4/10/96. These were summarized in a letter by the <br />Division on 9/10/96. These items are outstanding because the operator had not been given <br />access by the landowner to correct. The current maximum affected acreage is 20 acres, which <br />presently is the approximate size of the operation. The disturbance is situated on steeply <br />sloping ridges along the north and south sides of Wildcat Gulch, an ephemeral tributary of <br />Deer Creek. <br />The inspection proceeded from the sediment pond near the entrance gate, up the paved access <br />and haul road, by the scale, and by the first three mine benches to the topsoil stockpile. <br />The erosion rills were inspected on this stockpile. The operator agreed to grade the bottom <br />of the pile where the rills are deepest. A temporary seed mix will need to be spread on the <br />regraded area. The upland end of the culvert in Wildcat Gulch was identified at the base of <br />the topsoil stockpile. The three highest highwalls were inspected. Runoff control <br />structures observed included the check dam along Bench 2, the culvert by the topsoil <br />stockpile, and the sediment pond near the entrance gate. <br />The operator agreed to supply a map of the mine showing the height of the highwalls and a <br />copy of the storm water management plan so that the Division could evaluate the runoff <br />control features for the site. <br />I 4 E Contact Address <br />NAME Mike Sheahan <br />OPERATOR Western Mobile <br />STREET 1400 W. 64th Ave., P.O.
