(Page 2)

MINE ID H OR PROSPECTING ID 4k M-83-116
INSPECTION DATE 6/16/98 INSPECTOR'S INITIALS AJW
OBSERVATIONS
This was a normal monitoring inspection conducted by Tony Waldron of DMG. The site is a
gravelly upland located approximately 6 mile north and 1 mile east of Kornman, Colorado in
Prowers County.
The site did not have a mine identification sign located at its mine entrance and not allof
the permit boundary was clearly marked in the field. Both of these items are being cited as
problems along with appropriate corrective actions on page three of this report. Limited
mining has occurred at this site recently and much of the pit has started to establish native
volunteer vegetation. The operator's representative indicated that Kiowa County may want to
close this pit soon. Most of the resloped and revegetated areas could be released in their
current condition without additional reclamation. However, there are areas of the pit that
have been disturbed more recently to remove gravel and have not been backsloped or regraded.
Obviously, these areas will need to be regraded, topsoiled and seeded before final
performance warranty release could occur. This concluded the inspection.
I & E Contact Address
NAME Rodney Hopkins
OPERATOR Kiowa County
STREET P.O.
