:' • (Page 2) • <br />MINE ID # OR PROSPECTING ID #: M-~nnn-m~ <br />INSPECTION DATE: '~-n~-n1 INSPECTOR'S INITIALS: ~_ <br />The Division conducted a monitoring inspection of the site on 3-06- 01 while in the are conducting other <br />inspections. The main purpose of the inspection was to make sure the access road culverts were properly <br />maintained and were as reported in the permit application. The area, prior to the operator obtaining a <br />permit, was prone to erosion, which at times displaced the small culverts that were in place, with major <br />embankment failure. The operator had agreed to replace these culverts with two 24" culverts and stabilize <br />the embankment in the permit application. Since these culverts route the drainage for the small ephemeral <br />stream, the Division wanted to make sure these culverts were properly in place prior to spring runoff. <br />Operator was notified of the pending inspection but was unable to attend. Two corrugated 24 inch culvert <br />were in place embankment was stable no sign of erosion noted. Permit sign was properly displayed. A large <br />dozer was working the access road to the main pit area, north of the 42" road culverts, at the time of the <br />inspection.
